Job Summary

A company is looking for a Registered Nurse RN Case Manager.

Key Responsibilities

• Manage individual patients across the healthcare continuum to achieve optimal outcomes
• Collaborate with the interdisciplinary team to develop and manage patient discharge plans
• Educate healthcare team members on case management and managed care concepts

Required Qualifications

• Bachelor's degree in case management or healthcare
• Current Registered Nurse (R.N.) license in the state of practice
• 3-5 years of clinical experience in care management or acute/home care settings
• Basic Life Support (BLS) certification for acute care assignments
• Experience as a Case Manager in an acute care or home care setting within the past 12 months
