Colorado Licensed RN Case Manager

Job Summary

A company is looking for an RN Case Manager Expert (IKC).

Key Responsibilities

• Coordinate and facilitate quality care for ESRD and CKD patients, minimizing healthcare delivery fragmentation
• Implement intensive care coordination to stabilize high-risk patients and reduce hospital readmissions
• Collaborate with healthcare team members, patients, and families to ensure effective care management

Required Qualifications, Training, and Education

• A.D.N degree from an accredited nursing school required; B.S.N preferred
• Current Registered Nurse (RN) license in the state of practice required
• Minimum of five (5) years' experience in clinical nursing required
• Minimum of three (3) years' experience in renal nursing preferred
• Current CPR certification required
