Colorado Licensed RN Triage Nurse

Job Summary

Providing remote clinical support for outpatient practices, the full-time Colorado Licensed RN Triage Nurse will perform telephonic triage, manage patient messages in the electronic health record, and ensure patient-centered care through effective communication and collaboration with the care team.

Key responsibilities

• Perform remote clinical triage using evidence-based protocols to assess symptoms and determine urgency
• Manage high-volume patient messages, providing nursing guidance and routing requests as needed
• Coordinate care and support value-based care initiatives through timely appointments and follow-up outreach

Required qualifications

• Current Registered Nurse (RN) license in the state of practice
• Bachelor of Science in Nursing (BSN) from an accredited institution
• Demonstrated clinical nursing experience in chronic disease management
• Understanding of disease management processes and barriers to care delivery
• Basic computer skills and knowledge of Microsoft Office software
