Job Summary

A company is looking for a Social Worker (IKC).

Key Responsibilities:

• Provide behavioral health support to patients and families navigating kidney disease
• Coordinate resources and connect patients with community support services
• Assist with advance care planning and educate on palliative and hospice care options

Qualifications:

• Master's degree in Social Work from an accredited institution
• State licensure as a Licensed Clinical Social Worker (LCSW) required
• 2-4 years of clinical experience, preferably in healthcare settings
• 2 years of renal experience preferred
• Bilingual preferred
