Job Summary
To provide comprehensive telehealth services, a part-time Colorado Licensed Telehealth Provider will evaluate and treat patients through personalized hormone replacement therapy, medical weight management, and preventive healthcare, working remotely with required availability on Mondays and Tuesdays, plus one additional day based on business needs.
Key responsibilities
- Evaluate and treat patients via telehealth, focusing on hormone optimization and wellness care
- Build long-term relationships with patients while practicing evidence-based medicine in a collaborative environment
- Complete the hiring and credentialing process promptly, including verification of licensure and certifications
Required qualifications
- Active Colorado Family Nurse Practitioner (FNP) license or Colorado Physician Assistant license
- Current Colorado DEA registration at the time of application
- Current National Provider Identifier (NPI) and applicable board certification
- No current or pending malpractice claims or disciplinary actions
- Experience or strong interest in hormone replacement therapy and preventive medicine preferred
