Job Summary
A company is looking for a Telephonic Care Navigator to provide telephonic outreach and support to members.
Key Responsibilities:
- Conducts telephonic outreach to members, educating them on care gaps and scheduling appointments
- Links members to community resources addressing social determinants of health
- Maintains confidentiality of health information and meets quality and performance metrics
Required Qualifications:
- High School Diploma / GED or equivalent work experience
- Must be 18 years of age or older
- 1+ years of healthcare, insurance, or community outreach experience
- 1+ years of call center or telephonic customer service experience
- Experience using Microsoft Office applications (Word, Excel, Outlook)
