Job Summary

A company is looking for a Telephonic Care Navigator to provide telephonic outreach and support to members.

Key Responsibilities:
  • Conducts telephonic outreach to members, educating them on care gaps and scheduling appointments
  • Links members to community resources addressing social determinants of health
  • Maintains confidentiality of health information and meets quality and performance metrics
Required Qualifications:
  • High School Diploma / GED or equivalent work experience
  • Must be 18 years of age or older
  • 1+ years of healthcare, insurance, or community outreach experience
  • 1+ years of call center or telephonic customer service experience
  • Experience using Microsoft Office applications (Word, Excel, Outlook)
