Job Summary
A company is looking for a Telephonic Nurse Practitioner (Part-Time) with a Colorado License.
Key Responsibilities:
- Provide telephonic/telehealth care and coordination for members across various settings
- Utilize electronic medical records to deliver quality care and document assessments and findings
- Coordinate care transitions and monitor member needs, adjusting care plans as necessary
Required Qualifications:
- Active and unrestricted Nurse Practitioner license in Colorado and the state of residence
- Graduate of an accredited Master of Science in Nursing or Doctor of Nursing Practice program
- Board certification through a national board in relevant Nurse Practitioner specialties
- Current DEA licensure or ability to obtain post-hire as per state regulations
- Proficient in using electronic medical records for documentation
