Job Summary

A company is looking for a Telephonic Nurse Practitioner with a Colorado license.

Key Responsibilities:

• Provide telephonic and telehealth care to patients, coordinating with caregivers and facilities
• Deliver quality care and manage medical and behavioral conditions effectively
• Coordinate care transitions and monitor patient needs while facilitating adjustments to care plans

Required Qualifications:

• Active and unrestricted Nurse Practitioner license in Colorado and the state of residence
• Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
• Board certified as a Nurse Practitioner through a national board
• Current DEA licensure or ability to obtain post-hire
• Proficient computer skills for documenting medical information
