Job Summary
A company is looking for a Telephonic Nurse Practitioner (Part-Time) with a Colorado License.
Key Responsibilities:
- Provide telephonic and telehealth care, coordinating with patients, caregivers, and healthcare providers
- Deliver quality care and manage both medical and behavioral conditions, ensuring proper documentation and compliance
- Coordinate care transitions and facilitate adjustments to care plans as needed, while maintaining effective communication with all parties involved
Required Qualifications:
- Active and unrestricted Nurse Practitioner license in Colorado and the state of residence
- Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
- Board certification as a Nurse Practitioner in relevant specialties
- Current DEA licensure or ability to obtain it post-hire
- Proficient in using electronic medical records for documentation
