Colorado Licensed Triage Nurse

Job Summary

A company is looking for a Registered Nurse Triage and Transition.

Key Responsibilities

• Identify and assess patients for proactive interventions and develop individualized care plans
• Coordinate care plans and evaluate their effectiveness, modifying as necessary
• Conduct remote nursing assessments and provide patient support through care navigation and education

Required Qualifications

• Current Registered Nurse (RN) license in state of practice
• Bachelor of Science in Nursing (BSN) from an accredited institution
• Clinical nursing experience in chronic disease management
• Understanding of disease management processes and barriers to care delivery
• Basic computer skills and knowledge of Microsoft Office software
