Job Summary

Offering a fully remote opportunity, the full-time Colorado Licensed Triage Nurse will manage patient care transitions and triage services through telephone and digital communication, ensuring continuity of care and connecting patients to necessary resources.

Key responsibilities

• Conduct patient identification and assessments to develop individualized care plans and recommendations
• Coordinate care plan management and evaluate effectiveness while addressing social determinants of health
• Perform acute symptom triage and provide patient support through health coaching and education

Required qualifications

• Current Registered Nurse (RN) license in Colorado
• Bachelor of Science in Nursing (BSN) from an accredited institution
• Clinical nursing experience in chronic disease management
• Understanding of disease management and barriers to care delivery
• Basic computer skills and knowledge of Microsoft Office software
