Job Summary

Required Qualifications

• 3+ years of clinical practical experience
• 2+ years of care management or home health care coordination experience
• Active and unrestricted Colorado RN licensure or compact license
• Associate's Degree in Nursing (REQUIRED)
• Bachelor's Degree (PREFERRED)
