Job Summary

Key Responsibilities
- Completes telephonic outreach to members and assists with scheduling provider appointments
- Educates members on gaps in care and links them to community resources
- Maintains confidentiality of health information and meets performance metrics
Required Qualifications
- High School Diploma / GED or equivalent work experience
- At least 1 year of call center or telephonic customer service experience
- 1 year of professional experience in an office setting using telephone and computer
- 1 year of experience in healthcare, insurance, or social work/community outreach
- Ability to work full-time and attend a 3-week training period without time off
