Job Summary

A company is looking for a Community Connector, Rural at Heart.

Key Responsibilities

• Implement evidence-based blood pressure control practices and provide related education
• Support community partners and host sites to improve health outcomes and promote nutrition security initiatives
• Deliver health education and recruit, train, and coordinate volunteers to achieve community impact goals

Required Qualifications

• High school diploma or equivalent required; college coursework in public health or related fields preferred
• Proven track record of organizational skills and ability to work independently and in teams
• Commitment to improving rural health outcomes
• Intermediate proficiency with Microsoft Outlook and Word
• Able to lift and/or move large objects and access reliable internet when working remotely
