Community Health Manager
Job Summary
Overseeing the implementation of State Rural Health Transformation grants, the full-time Community Health Manager will strengthen brain health and care through community engagement efforts, build partnerships with health systems, and supervise a team focused on dementia care initiatives.
Key responsibilities
- Lead the execution of rural health care transformation strategies, ensuring compliance with grant requirements and goals
- Develop and maintain partnerships with healthcare providers and community organizations to enhance dementia care access
- Supervise and support the team responsible for workforce development and community resource coordination initiatives
Required qualifications
- Bachelor's degree in public health, social work, health administration, nursing, gerontology, or a related field; Master's degree preferred
- 3-5 years of experience in public health, community health, or related fields, with 5+ years in leadership roles
- Experience working with Federally Qualified Health Centers (FQHCs) and community-based organizations
- Proven success in leading workforce development initiatives and care coordination models
- Strong background in stakeholder engagement and cross-sector collaboration
