Community Health Navigator

Job Summary

To support a growing pediatric practice, the part-time hourly Community Health Navigator will assist patients in obtaining post-hospital care through telephonic and in-person support while collaborating with interdisciplinary teams to address social determinants of health.

Key responsibilities

• Outreach to patients discharged from emergency rooms to ensure continuity of care and identify needs related to social determinants of health
• Coordinate with facility discharge planners to eliminate barriers to after-hospital resources and care
• Deliver resources to families and support in-person/community needs related to care coordination and discharge support

Required qualifications

• Bachelor's degree required
• Bilingual (Spanish) preferred
• National CHW certification preferred
• CPR certification preferred
• 3+ years of experience in a healthcare setting, preferably in an ER setting
