Job Summary
To support members with complex health needs, the full-time remote Community Health Partner will manage a caseload, collaborate with RN Care Managers, and develop care plans addressing social determinants of health.
Key Responsibilities
- Collaborate with RN Care Managers to support members with complex medical and behavioral health needs
- Manage a caseload of members, identifying and addressing social, behavioral, and environmental barriers to optimal health
- Conduct assessments and refer members to appropriate behavioral health programs as needed
Required Qualifications
- 2+ years of experience in care coordination, community health, health promotion, or a related field
- Strong critical thinking and problem-solving skills
- Experience with documentation and effective communication
- Demonstrated ability to build rapport with members and navigate emotional barriers
- Proven track record of collaboration and team effectiveness
