A company is looking for a Senior Program Manager - Community Health.

Key Responsibilities

• Lead and sustain community health initiatives aligned with organizational goals
• Develop strategies to enhance community-clinical linkages for improved population health
• Manage complex community health programs from planning to evaluation and continuous improvement

Required Qualifications

• Bachelor's degree in public health, community health, public administration, or related field; master's degree preferred
• Seven or more years of relevant industry experience with strong leadership skills
• Experience in facilitating diverse community-based partnerships
• Detailed knowledge of population health and public health research
• Experience with data management and Electronic Health Records
