Job Summary

A company is looking for a Community Health Worker.

Key Responsibilities

• Provide culturally appropriate complex care management in a primary care setting
• Assist patients in overcoming barriers and navigating the healthcare system
• Support patients in making meaningful progress toward health-related goals over a 6-month period

Required Qualifications

• High school diploma or GED preferred
• Valid driver's license and clean driving record if driving is required
• Two years of experience in a social service or healthcare setting preferred
• Must be a resident of the community being served with knowledge of local resources
• Basic computer skills preferred
