Community Health Worker

Job Summary

Providing essential services to patients and their families, the full-time Community Health Worker will coordinate case management and social care services while working onsite with the option for one remote day per week after a six-month probationary period.

Key responsibilities

• Coordinate case management and social care services for patients and families
• Assist patients in navigating community resources and public/government benefits
• Promote family-centered care principles in all interactions

Required qualifications

• High school diploma/GED or four years of related education and/or work experience
• Experience with community resource navigation and social-care support
• Familiarity with health education practices
• Bilingual skills are preferred
• Experience with public/government benefits is preferred
