Community Health Worker

Job Summary

As a remote, part-time Community Health Worker, the Care Advocate will support Medicare patients by helping them understand their care plans, schedule appointments, and navigate insurance while connecting them to community resources.

Key responsibilities

• Serve as the primary point of contact for patients post-intake visit, assisting with care plan comprehension and appointment scheduling
• Resolve navigation challenges related to referrals, appointment logistics, and provider communications
• Document all patient interactions and support activities accurately and in a timely manner

Required qualifications

• Community Health Worker (CHW) certification or equivalent experience in community health or patient navigation
• At least 2 years of experience in patient- or community-facing support roles
• Working knowledge of the U.S. healthcare system and Medicare
• Ability to work independently while managing a caseload and maintaining documentation
• Access to reliable computer and internet, along with a quiet space for patient calls
