Community Health Worker

Job Summary

To support the healthcare team, the full-time Community Health Worker will assist patients and community members in navigating the healthcare system, providing education on healthy behaviors and facilitating access to resources in the greater Charlotte, NC area, primarily working remotely.

Key responsibilities

• Provides direct services, including health education and prevention programs, to clients
• Acts as a liaison between clients and healthcare providers, ensuring effective communication and follow-up
• Facilitates access to community resources and conducts home visits or community events to support client needs

Required qualifications

• High School Diploma or GED required; Bachelor's degree preferred
• 1-2 years of experience in community health or related fields
• Completion of required certifications or continuing education in community health within the first year of employment
• Valid driver's license and reliable transportation for patient outreach
• In-depth knowledge of community resources and health services
