Community Health Worker

Job Summary

To support adolescents and young adults with childhood-onset chronic conditions, the part-time Community Health Worker will provide peer coaching and guidance remotely, helping families navigate healthcare systems and achieve personal health goals.

Key responsibilities

• Provide peer health and wellness coaching through telephone and virtual platforms, utilizing lived experience to promote engagement and self-management
• Support patients in navigating healthcare systems and community resources, helping them understand available services and overcome barriers to care
• Maintain accurate documentation of coaching encounters and patient progress in accordance with program requirements

Required qualifications

• High school diploma or equivalent
• One year of experience in a position involving patient, family, public, or community contact
• Knowledge of and experience navigating youth-serving systems related to special healthcare needs
• Ability to build trusting relationships with patients, families, and healthcare team members
• Basic computer skills for documenting services and interactions accurately
