Community Health Worker Certification

Job Summary

A company is looking for a Community Health Worker to support patients with complex chronic conditions through advocacy, resource connection, and care coordination.

Key Responsibilities

• Screen patients for Social Determinants of Health needs and connect them with appropriate community resources
• Coordinate care by facilitating communication between patients and their healthcare providers
• Provide wellness coaching and assist patients in developing practical life skills

Required Qualifications

• Community Health Worker Certification, CMA, RMA, or an Associate's degree in health sciences or related field with 3+ years of healthcare experience
• Experience in identifying community resources and assisting patients in accessing them
• Experience working within an interdisciplinary healthcare team
• Comfortable advocating for patients with internal and external stakeholders
• Prior experience with complex patients and/or underserved populations
