Required Qualifications

Community Health Worker Certification, CMA, RMA, or an Associate's degree in health sciences or related field with 3+ years of healthcare experience
Experience identifying community resources and assisting patients in accessing them
Experience working in an interdisciplinary healthcare team
Prior experience with complex patients or underserved populations
Comfortable advocating for patients with various stakeholders
