Community Referral Coordinator

Job Summary

To support a growing team in Lane County, OR, the full-time Community Referral Coordinator will manage patient referrals, schedule appointments, and build relationships with community partners while working Monday through Friday in a fast-paced environment.

Key responsibilities

• Process and schedule inbound patient referrals from community partners using CRM and EHR systems
• Document patient information and follow up on inquiries to ensure smooth communication and care coordination
• Develop and maintain trusted relationships with community partners to facilitate effective referrals and support

Required qualifications

• Must reside in the Eugene, OR area
• High School Diploma or GED
• One year of customer service experience
• Experience with personal computers, word processing, and electronic spreadsheets
• Knowledge of chemical dependency and its effects on individuals, families, and communities
