Job Summary

To support a growing healthcare practice, the full-time Community Resource Guide will engage patients remotely and in-person, facilitating care navigation, community resource connections, and addressing barriers to care to improve health outcomes.

Key responsibilities

• Building trusting relationships with patients, families, and healthcare providers to facilitate care coordination
• Engaging high-risk patient populations to address health needs and assist with care plans and follow-ups
• Identifying and assessing patients' medical, behavioral, and social needs to support whole-person care

Required qualifications

• At least 3 years of experience in patient navigation, community resource navigation, or care coordination
• Experience working directly with patients with complex medical and social needs
• Familiarity with EHR systems and documentation platforms
• Knowledge of medical terminology and primary care workflows
• Demonstrated ability to work effectively in a remote environment
