Community Resource Navigator

Job Summary

A company is looking for a Community Resource Navigator, Remote (PA/NJ/DE).

Key Responsibilities:

• Conduct outreach to members with potential SDOH-related needs and perform psychosocial assessments
• Educate members and families on available benefits, programs, and community resources
• Coordinate referrals to internal programs and external agencies

Qualifications:

• Bachelor's degree, preferably in social work, psychology, or human services
• Minimum of 3-5 years of experience addressing social determinants of health and working with vulnerable populations
• Proficiency in documentation and use of care management systems
• Knowledge of community resources and social service systems
