Community Resource Navigator

Job Summary

To enhance community health outcomes, the fully remote Community Resource Navigator will conduct outreach, assess psychosocial needs, and coordinate services for members facing social determinants of health challenges in Pennsylvania, New Jersey, and Delaware.

Key Responsibilities:

• Conduct outreach to members with potential SDOH-related needs and perform psychosocial assessments
• Educate members and families on available benefits, programs, and community resources
• Coordinate referrals to internal programs and external agencies while documenting all interactions in the managed care system

Required Qualifications:

• Bachelor's degree, preferably in social work, psychology, or human services
• Minimum of 3-5 years of experience addressing social determinants of health and working with vulnerable populations
• Strong interpersonal, communication, and organizational skills
• Proficiency in documentation and use of care management systems
• Knowledge of community resources and social service systems
