Compact Licensed Bilingual LPN

Job Summary

Supporting patients enrolled in chronic care management programs, the full-time Compact Licensed Bilingual LPN will conduct consultations, educate patients on chronic conditions, and maintain accurate documentation while working remotely.

Key Responsibilities:

• Complete phone consultations with patients, providing support and education about their chronic conditions
• Educate patients on health conditions and empower them to manage their chronic illnesses effectively
• Maintain accurate patient records, ensuring all interactions and care plans are documented per protocol

Required Qualifications:

• A current, valid, and in good standing Multistate/Compact Nursing License (LPN/LVN)
• 3+ years of experience in primary care practice, cardiology, internal medicine, home care, or chronic care management
• Ability to communicate fluently in Spanish
• Proficiency in using electronic health records (EHR) and care management software
• Reliable internet connection and proficiency with virtual communication tools (e.g., Zoom, Slack)
