Compact Licensed Care Management RN

Job Summary

A company is looking for a Registered Nurse - APCM - Compact License Required.

Key Responsibilities

• Proactively monitor patient vitals, symptoms, and clinical data to identify care gaps
• Develop and manage individualized care plans aligned with physician recommendations
• Engage with patients to address concerns and provide clinical support through virtual follow-ups

Required Qualifications

• Active Multi-State RN Compact License
• Associate Degree in Nursing Science
• 5+ years of recent clinical experience as an RN in an ICU/ER setting preferred
• Prior experience in chronic care management is a plus
• EMR experience, preferably in Athena and EPIC
