Compact Licensed Care Navigator

Job Summary

A company is looking for a Care Navigator (Remote LPN).

Key Responsibilities:

• Complete phone consultations with patients, providing support and education about chronic conditions
• Educate patients on health conditions and empower them with strategies to manage their care
• Maintain accurate patient records and respond to remotely transmitted patient data

Qualifications:

• Current, valid Multistate/Compact Nursing License (LPN/LVN)
• 3+ years of experience in primary care, cardiology, internal medicine, home care, or chronic care management
• Proficiency in using electronic health records (EHR) and care management software
• Reliable internet connection and proficiency with virtual communication tools
• Additional state licenses may be required and reimbursed
