Compact Licensed Care Navigator

Job Summary

A company is looking for a Care Navigator (Remote LPN).

Key Responsibilities:

• Complete phone consultations with patients providing support and education about their chronic conditions
• Educate patients on health conditions and empower them to manage their chronic illnesses
• Maintain accurate patient records and document all interactions and care plans

Qualifications:

• Current, valid, and in good standing Multistate/Compact Nursing License (LPN/LVN)
• 3+ years of experience in primary care, cardiology, internal medicine, home care, or chronic care management
• Proficiency in using electronic health records (EHR) and care management software
• Reliable internet connection and HIPAA-compliant work area
• Experience with virtual communication tools (e.g., Zoom, Slack)
