Job Summary

Key Responsibilities

• Assess, plan, implement, and coordinate case management activities to evaluate members' medical needs
• Utilize clinical tools and apply clinical judgment to address complex health and social indicators
• Collaborate with stakeholders to overcome barriers and enhance members' overall wellness

Required Qualifications

• Registered Nurse with an unrestricted nursing license and multi-state/compact privileges
• 3+ years of clinical/nursing practice experience
• 1+ year of medical case management, care coordination, or discharge planning experience
• High-speed internet access for remote work
• 3+ years of experience with personal computers and MS Office Suite applications
