Job Summary

A company is looking for a Clinical Registered Nurse - Remote.

Key Responsibilities

• Provide telephonic support and coordinate with healthcare teams to manage total healthcare for participants
• Identify at-risk participants and implement interventions according to disease management guidelines
• Facilitate communication among multidisciplinary teams and ensure continuity of care for participants

Required Qualifications

• Current RN license for the state of employment and ability to obtain licensure in other states
• High School diploma and a Diploma, Associate's Degree, or Bachelor's Degree in Nursing (BSN preferred)
• Minimum of 3 years recent clinical experience
• Basic knowledge of physiology and chronic diseases relevant to nursing
• Basic computer skills and familiarity with a Windows operating system
