Compact Licensed LPN Care Navigator

Job Summary

To support patients enrolled in chronic care management and remote patient monitoring programs, the full-time Compact Licensed LPN Care Navigator will educate patients on managing chronic diseases, maintain accurate patient records, and communicate effectively with care teams while working remotely.

Key Responsibilities:

• Complete phone consultations with patients to provide support and education about chronic conditions
• Educate patients on health management strategies and assist them in achieving their provider-approved care goals
• Maintain accurate patient records and document all interactions and care plans per protocol

Required Qualifications:

• A current, valid, and in good standing Multistate/Compact Nursing License (LPN/LVN)
• 3+ years of experience in primary care practice, cardiology, internal medicine, home care, or chronic care management/remote patient monitoring
• Proficiency in using electronic health records (EHR) and care management software
• Reliable internet connection and HIPAA-compliant work area
• Empathy and a patient-centered approach to care
