Compact Licensed LPN Care Navigator

Job Summary

To support patients enrolled in chronic care management and remote patient monitoring programs, the full-time Compact Licensed LPN Care Navigator will provide education, support, and communication with patients and their care teams while working remotely from the United States.

Key Responsibilities:

• Complete phone consultations with patients, offering support and education about chronic conditions
• Educate patients on managing their health and assist them in setting and achieving goals aligned with care plans
• Maintain accurate patient records and document all interactions per protocol

Required Qualifications:

• Current, valid, and in good standing Multistate/Compact Nursing License (LPN/LVN)
• 3+ years of experience in primary care, cardiology, internal medicine, home care, or chronic care management
• Proficiency in electronic health records (EHR) and care management software
• Reliable internet connection and proficiency with virtual communication tools (e.g., Zoom, Slack)
• Empathy and a patient-centered approach to care
