Compact Licensed MTM Nurse

Job Summary

Compact Licensed MTM Nurse, responsible for delivering telephonic medication therapy management services remotely on a contract basis, requiring a Compact RN license and a minimum of two years of related experience.

Key Responsibilities

• Perform telephonic comprehensive medication reviews and clinical assessments for high-risk members
• Identify, document, and facilitate resolution of medication-related problems
• Create written summaries of member interactions and materials for prescriber offices

Required Qualifications

• Associate's Degree in Nursing and Compact Licensure in State of Practice
• Minimum of 2 years of related practice experience
• Active nurse license in a Compact Licensure State

Preferred Qualifications

• Bachelor's Degree in Nursing
• Case Care Management Certificate
• Previous experience delivering MTM using the Outcomes MTM Connect platform
• Strong clinical knowledge of chronic diseases and health promotion
