Job Summary

A company is looking for a Nurse Case Manager - Maternity (Remote-CST/MST/PST).

Key Responsibilities

Assess the needs of members and develop individualized care plans
Monitor care plans and coordinate with providers to ensure appropriate services are delivered
Provide education and referrals to community resources for members

Required Qualifications

Registered Nurse (RN) with a current, unrestricted compact license
Preferred college degree
Three or more years of experience in a Case Management role
Three or more years of clinical experience in OB/L&D or related area
CCM certification required within specified timeframe
