Job Summary

As a Care Navigator, the full-time Compact Licensed Physical Therapist will provide remote monitoring services to support patients in their home-exercise therapy programs, assist users in therapeutic exercises, and maintain records while collaborating with clinicians to enhance patient outcomes.

Key responsibilities

• Provide remote monitoring services to support patients' progress through home-exercise therapy
• Direct and assist users in therapeutic exercises, neuromuscular re-education, and functional training via the technology platform
• Maintain necessary records and provide ongoing feedback to evaluating clinicians regarding patient progress

Required qualifications

• Active PT license in a compact state
• Graduated from an accredited physical therapy program
• 2+ years of experience in musculoskeletal health is a plus
• Experience in behavioral modification; Health Coach Certification is a plus
• Tech-savvy and comfortable navigating multiple platforms (EHR, messaging, scheduling)
