Job Summary

A company is looking for a Compact Licensed Physical Therapist (Care Navigator) to support patient success through remote monitoring and intervention.

Key Responsibilities

• Provide remote monitoring services for patients in their home-exercise therapy program
• Assist users in therapeutic exercise and functional training via a technology platform
• Maintain records and provide feedback on patient progress while managing a caseload efficiently

Required Qualifications

• Graduated from an accredited physical therapy program
• Active Compact PT license
• 2+ years of experience in musculoskeletal health is a plus
• Tech-savvy with access to a newer model laptop and smartphone
• Experience in behavior change or health coaching is a plus
