Job Summary

As a Care Navigator, the full-time Compact Licensed Physical Therapist will provide remote monitoring services to support patients in their home-exercise therapy programs, guiding them through therapeutic exercises and functional training while maintaining records and collaborating with clinicians.

Key responsibilities

• Provide remote monitoring and support for patients progressing through home-exercise therapy
• Direct users in therapeutic exercise and functional training using the technology platform
• Maintain records and communicate patient progress to evaluating clinicians

Required qualifications

• Active PT license in a compact state
• Graduated from an accredited physical therapy program
• 2+ years of experience in musculoskeletal health is a plus
• Tech-savvy with experience in multi-platform environments
• Experience in behavioral modification; Health Coach Certification is a plus
