Job Summary

As a Care Navigator, the full-time Compact Licensed Physical Therapist will provide remote monitoring services, assist patients with therapeutic exercises, and maintain records to support musculoskeletal health while working in a collaborative, fully remote environment.

Key responsibilities

• Provide remote monitoring services to support patients through their home-exercise therapy program
• Direct users in therapeutic exercise and functional training via the technology platform
• Organize and manage a caseload while providing ongoing feedback to evaluating clinicians on patient progress

Required qualifications

• Active PT license in a compact state
• Graduated from an accredited physical therapy program
• 2+ years of experience in musculoskeletal health is preferred
• Experience in behavioral modification; Health Coach Certification is a plus
• Tech-savvy with the ability to navigate multiple platforms effectively
