Job Summary

As a full-time remote Compact Licensed Physical Therapy Assistant, the Care Navigator will provide essential support to patients by monitoring their progress through home-exercise therapy programs, assisting with therapeutic exercises, and maintaining communication with clinicians regarding patient outcomes.

Key responsibilities

• Provide remote monitoring services to support patients in their home-exercise therapy programs
• Direct and assist users in therapeutic exercise and functional training through a technology platform
• Maintain necessary records and provide ongoing feedback to clinicians on patient progress

Required qualifications

• Active PTA license in a compact state
• Graduated from an accredited physical therapy program
• 2+ years of experience in musculoskeletal health preferred
• Proficiency in using multiple technology platforms (EHR, messaging, scheduling)
• Experience in behavioral modification or Health Coach Certification is a plus
