Job Summary

As a Care Navigator, the full-time Compact Licensed Physical Therapy Assistant will provide remote monitoring services, assist patients with therapeutic exercises, and maintain communication with clinicians to support patient recovery and musculoskeletal health.

Key responsibilities

• Provide remote monitoring services to support patients in their home-exercise therapy program
• Direct and assist users in therapeutic exercises and functional training through a technology platform
• Maintain necessary records and provide ongoing feedback to clinicians regarding patient progress

Required qualifications

• Active PTA license in a compact state
• Graduated from an accredited physical therapy program
• 2+ years of experience in musculoskeletal health is a plus
• Tech-savvy with the ability to navigate multiple platforms effectively
• Experience in behavioral modification or Health Coach Certification is a plus
