Compact Licensed Practical Nurse

Job Summary

To support innovative patient care, the full-time Remote Compact Licensed Practical Nurse will engage with patients on the RPM365 platform, assist with device setup, and ensure adherence to care plans through high-volume phone outreach.

Key responsibilities

• Serve as the primary contact for patients using the RPM365 platform
• Monitor patient data and schedule monthly check-ins to promote adherence to care plans
• Educate patients and notify healthcare providers of critical alerts as necessary

Required qualifications

• Active compact/multistate nursing license
• Knowledge of chronic conditions and related monitoring devices
• Proficiency with technology, computer platforms, and smartphones
• Experience in patient outreach and engagement
• Highly organized and independently motivated
