Compact Licensed Registered Nurse

Job Summary

A company is looking for a Registered Nurse (Remote) to support a team of clinicians in delivering complex care plans.

Key Responsibilities

• Provide telephonic care management and triage for patients and families
• Conduct chronic and transitional care management outreach, assessment, and intervention
• Coordinate clinical care with other providers and manage medication reconciliation

Required Qualifications

• Active Compact RN licensure required; additional licenses in CA, CT, or MI are a plus
• Minimum of 3 years clinical experience, preferably in a centralized care management team
• Prior healthcare experience with chronically ill patients preferred
• Experience in ER, Critical Care, or Home Health is a plus
• Strong customer-service orientation and exceptional organizational skills
