Compact Licensed Registered Nurse

Job Summary

A company is looking for a Registered Nurse (Remote) to support a team of clinicians in delivering complex plans of care.

Key Responsibilities

• Provide telephonic care management and triage for patients and families, addressing urgent issues and making proactive outreach calls
• Conduct chronic and transitional care management to identify and close gaps in patient care
• Coordinate clinical care with other providers and manage medication reconciliation and administration tasks

Required Qualifications

• Active Compact RN licensure required; additional licenses in CA, CT, or MI are a plus
• Minimum of 3 years clinical experience, preferably in a centralized care management environment
• Prior healthcare experience with chronically ill patients preferred
• Experience in ER, Critical Care, or Home Health is advantageous
• Strong organizational skills and ability to multi-task in a fast-paced environment
