Compact Licensed Registered Nurse

Job Summary

To support a team of clinicians in delivering complex care plans, the full-time Remote Compact Licensed Registered Nurse will provide telephonic care management, coordinate post-discharge services, and ensure safe transitions for high-risk patients.

Key responsibilities:

• Manage telephonic care for patients and families, addressing urgent issues and conducting proactive outreach
• Coordinate chronic and transitional care management to identify and close gaps in patient care
• Facilitate clinical care coordination with providers, including appointment scheduling and service authorizations

Required qualifications:

• Active Compact RN licensure required, with additional licensure in CA, CT, or MI preferred
• Minimum of 3 years clinical experience, ideally in a centralized care management environment
• Experience with chronically ill patients and in ER, Critical Care, or Home Health settings is a plus
• Strong customer-service orientation and exceptional organizational skills
• Familiarity with Athena EMR is advantageous
