Job Summary

A company is looking for a Triage Support RN to provide remote triage services for Medicare patients with chronic conditions.

Key Responsibilities

• Provide non-face-to-face telephone triage and care planning for patients
• Collaborate with healthcare teams and document encounters in patient charts
• Educate patients and manage urgent care queues to assist in reaching health goals

Required Qualifications

• Current COMPACT RN license
• Willingness to obtain multi-state licenses
• Familiarity with chronic care conditions and related medications
• Ability to follow clinical triage protocols and document accurately
• Willingness to work an on-call rotating schedule
